*Please note that trainings are available to all districts. If a district has paid for Extended Services, the above training is available at a rate of $25/person for a half day and $37.50/person for a full day. If a district has not paid for Extended Services the above training is available at a rate of $50/person for a half day and $75/person for a full day.
Consultation Fees:
If a consultant is invited to a school to provide technical assistance to teachers where no formal training is done a consultation fee of $50 for a half day and $100 for a full day will be assessed for those participating in the Extended Services Plan. For districts not participating in the Extended Services Plan the rate will be $100 for a half day and $200 for a full day.
Your district will be billed monthly for all trainings/services provided.
I authorize the training indicated above. I understand that my district is financially responsible for the cost of this training and will be billed for these services upon their completion. I also understand that payment will be due within 30 days of receipt of the invoice and that if any registered attendees are unable to attend notice must be given at least 24 hours in advance; otherwise the district will be billed for the training.
My signature below acknowledges my authorization to enter into this agreement and that I am responsible for the payment of the service(s) provided to my district.
