Directions: 

Step 1: Once your team has agreed upon a cluster area (those in colored bars below) and the number one priority predictor (shown under colored bars), click on that predictor and begin ranking its characteristics and submit.

Step 2: Your rankings will be emailed to you and you can begin to build an action plan with your team (with help from your Transition Consultant as needed). 

Step 3: Submit your action plan and a copy will be emailed to the address you indicated on the form. Your Transition Consultant will follow up with you as well to ensure follow-through of the plan.
